OPTIMUM REWARDS TERMS AND CONDITIONS
The following terms and conditions (these "Terms and Conditions") contain important information regarding the CSC Holdings, Inc.'s ("Sponsor") Optimum Rewards program (the "Program"). Please read the following terms and conditions carefully. These Terms and Conditions are a binding agreement between you and Sponsor and will govern your participation in any and all Program offers.
Being eligible for the Program entitles Cablevision residential subscribers ("Subscribers") to take advantage of certain Program offers and/or discounts that will change from time to time (the "Program Offers/Discounts"), and which may be made available for limited times, all as determined by Sponsor in its sole discretion. You can find out about available Program Offers/Discounts by checking www.optimumrewards.com and eligible Subscribers are encouraged to check back frequently to find out what new Program Offers/Discounts may be available. As part of the Program, you will receive an Optimum Rewards Card. You may need to show the Optimum Rewards Card and a valid photo ID in order to receive certain Program Offers/Discounts. Program Offers/Discounts that are merchandise and/or special events are available only while supplies last or as space permits Sponsor reserves the right to modify these Terms and Conditions at any time, without notice to you, so it is important to check the Terms and Conditions periodically. Participation in the Program and/or redemption of Program Offers/Discounts is considered acceptance of these Terms and Conditions and any modified terms included therein. Sponsor may, in its sole and absolute discretion, cancel, change, suspend or modify any aspect of the Program and/or any Program Offer/Discount at any time, including the availability of any Program Offer/Discount, without notice.
ELIGIBILITY:
To be eligible for Program Offers/Discounts, you must be a residential Subscriber in good standing who is a legal resident of New York, New Jersey or Connecticut and who has (or who upgrades to) all three of the following services: (a) Optimum Online service; (b) Optimum Voice service; and (c) iO TV service (with at least Family Cable service). To be eligible for Program Offers/Discounts, you must have (or upgrade) to all three of the above services, and your account must be current and in good standing with no unpaid balances due. Cancellation or downgrade of any of the three required services may immediately void, alter or modify the availability of some or all Program Offers/Discounts. Employees of Sponsor, Sponsor affiliates and their immediate family members living in the same household of Sponsor are not eligible for Program Offers/Discounts. Program Offers/Discounts may not be combined with certain other offers, including but not limited to Double Play or Triple Play offers. Subscribers who receive Senior Citizen discounts or Seasonal discounts are not eligible for Program Offers/ Discounts.
TERMINATION:
Sponsor may, in its sole and absolute discretion, cancel, change, suspend or modify any aspect of the Program and/or any Program Offer/Discount at any time, including the availability of any Program Offer/Discount, without notice. Sponsor may, in its sole and absolute discretion, terminate or suspend any Subscriber's participation in the Program for breach of these Terms and Conditions, or for taking any actions that are inconsistent with the intent of these Terms and Conditions. Sponsor shall be the sole determiner in cases of suspected abuse, fraud, or breach of these Terms and Conditions or intent of these Terms and Conditions. Any decision Sponsor makes relating to termination or suspension of any Subscriber's participation in the Program shall be final and binding in all respects.
GENERAL:
Subscribers may only take advantage of one (1) Program Offer/Discount at a time. Additional fees apply for upgrades, installation and/or number porting. All terms and conditions of Optimum Online service, Optimum Voice service and iO TV service apply. All terms and conditions of individual Program Offers/Discounts apply. Specific terms and conditions of individual Program Offers/Discounts are available at www.optimumrewards.com. The Program and/or the Program Offers/Discounts are void where prohibited by law. Sponsor is not responsible for any failures or circumstances affecting, disrupting or corrupting the Program of the Program Offers/discounts in any manner including without limitation, Subscribers' ability to participate in the Program or any Program Offers/Discounts. Sponsor is not responsible for unavailable network connections; failed, incomplete, garbled or delayed computer transmissions; online failures; hardware, software or other technical malfunctions or disturbances; or any other communications failures or circumstances affecting, disrupting or corrupting the Program in any manner and Sponsor is also not responsible for any damage to any Subscriber's computer occasioned by participation in the Program or downloading of any information necessary to participate in the Program. By participating in the Program, Subscribers release Sponsor, its parent, subsidiaries, affiliates, suppliers, and agents from any and all liability for any loss, harm, damages, cost or expense, including without limitation property damages, personal injury and/or death, arising out of participation in the Program and the use of or participation in any Program Offers/Discounts. Sponsor makes no warranty in any respect as to any merchandise or service available within the Program. Under no circumstance will merchandise or any service be refunded or exchanged for other Program Offers/Discounts or cash. Program Offers/Discounts may be taxable, depending on the value of the item and the federal, state, and local tax laws applicable to you. You are solely responsible for reporting such items on your tax returns and paying any associated tax liability. You may not assign or transfer any Program Offers/Discounts.
Subscribers are responsible for maintaining the confidentiality of their username and password. Sponsor has no control over the Internet and cannot prevent the interception of messages by unauthorized parties or guarantee that such unauthorized parties may not be able to decrypt encrypted messages. Accordingly, under no circumstances will Sponsor be responsible for any loss or damage incurred by any Subscriber as the result of unauthorized interception or decryption of information transmitted to or by Sponsor. Subscribers must immediately notify Sponsor of any known or suspected unauthorized use(s) of a Subscriber's account, or any known or suspected breach of security, including loss, theft, or unauthorized disclosure of Subscriber's password. Personal nonpublic information that we gather from you in connection with the Program or otherwise will be governed by our Privacy Policy, and your use of the Optimum Rewards website will otherwise be governed by Cablevision's Terms of use, available at http://www.optimum.com/terms.jsp
